School Physicals are by appointment only and need to be scheduled by the school nurse. Students whose physicals are on record for a prior sport and have not expired for the fall season (valid for one year to start of sport) only need a health history review done in the summer either by phone conference or in person. All forms for sports can be obtained from the nurse or on the Steinert website.

In order to have a school physical, the Request for School Medical Examination must be signed and returned to nurse prior to the exam. A screening must also be done prior to physical date. Any questions regarding these directions, please call 631-4150, Ext. 5.
